◉ 臨床上重要な事実

 

• 重度の精神疾患を有する人は、自らの安全と介護者やケアスタッフの両方を脅かす可能性のある暴力的で攻撃的なエピソードを経験しうる1。暴力的な出来事を最小限にするために、絶えず怒りや興奮の徴候や症状を観察すること、病棟の扉を施錠すること、攻撃的エピソードに対処するための落ち着きコントロールされた態度による接近法（ディエスカレーション技術）、患者と専門家の責任の明確な線引き(行動契約として知られている)の相互の合意を得るための個人とケアチーム間の交渉のような非薬理学的戦略が用いられてきた1-2。
• システマティックレビューの目的は、重度精神疾患患者における暴力と攻撃のインシデントを減らし、スタッフと介助者の戦略に対する満足度を改善するため、様々な封じ込め戦略（例えば、定期的なスーパービジョンや病棟の扉の施錠、ディエスカレーション技術、行動契約のような）の効果を比較することである1。（レベル１）
• ガイドラインは、医療従事者が医療現場における暴力と攻撃性への管理に関するトレーニングを受けるべきであると述べている2。（レベル５）
• ガイドラインは、患者を罰し、痛みを与え、苦痛や屈辱を与えるための制限的介入の使用を推奨していない2。（レベル５）

 

◉ エビデンスの特性
